App or special document format for in field reports

I am looking for an app or a way to make a document template where my employees can fill out a "Daily Field Report" specific to our company. All I need is a document/spreadsheet template that can be edited using a smartphone. The template would include some spaces to fill in the project address and then some drop down lists with items like ambient weather conditions and in the list there would be sunny, cloudy, rain, etc.

Also the ability to add pictures on the end would be great.
