My Galaxy Note 4 won't charge properly

Hello! Can anyone help me out?
I got my galaxy note 4 back in September of 2014 and it's been working quite perfectly until recently.
Some current charging problems:

• If I connect my phone into the wall (when it has some battery life in it), it doesn't charge at all.
• If my phone is dead and I connect it to the wall, it doesn't charge and vibrates every 2 seconds with a blank screen.
• My phone will not connect to the car charger either.
